In 2008, the NRMP surveyed program directors about the factors programs use in ranking applicants (available at www.nrmp.org). Over 1,800 program directors representing 19 specialties responded. "Perceived interest in the program" as expressed by the applicant was rated 3.7 on a scale of 1 (not at all important) to 5 (very important).
Among the many ways you can express interest is by sending the program a letter of interest, also known as a letter of intent. These letters of intent should ideally be sent towards the end of January as programs near the end of the interview season. It is, of course, important that programs receive these letters before they have their final ranking meeting.
Can a letter of intent influence your position on a program's rank list? Not for every program but the literature does suggest that post-interview communication has an effect on ranking decisions at some programs. It is not unusual for applicants to be told to keep in touch if they have an interest in matching with the program.
